CHEMICAL PEELS FREQUENTLY ASKED QUESTIONS

WHAT IS A CHEMICAL PEEL?

Chemical peel repair skin damage, slows the aging process and rejuvenates the skin. In a chemical peel a chemical which is usually an acid is applied to the skin surface so that it produces a controlled burn of the skin surface. Dead and damaged skin peels off in a few days revealing younger looking, brighter smoother , more radiant skin. A chemical peel will treat photo-damaged skin, hyper-pigmentation, large pores, fine lines and uneven skin color and tone. It will also treat laxity dryness and roughness. It decreases acne scaring and can be used to control acne in some cases.

HOW DO CHEMICAL PEELS WORK?

A baby’s skin turns over in four weeks. As we age it takes longer for our skin to tunover. In thirties and forties it takes up to five to six weeks for the dead skin to shed. The skin looks dull, with appearance of fine lines and wrinkles, dryness, roughness, large pores and even skin tone. A Chemical peel hastens the process of skin turn over by exfoliating the top layers of the skin revealing younger, smooth, firm and radiant ski with even color and tone.

HOW MANY CHEMICAL PEELS DO I NEED?

The number of peels that are needed depend upon the amount of skin damage, type of skin and the depth of peeling. More deep peels will cause much more improvement but the recovery time is much longer and there are more possible side effects. Superficial peels are much more safe but the improvement in the skin is much less and you need more peels to get the same effect. Usually three to five of the medium strength peel will produce significant improvement in the skin. They can be done about three to six weeks apart.

WHAT IS THE RECOVERY PERIOD FOR CHEMICAL PEELS?

Recovery period depends upon the depth of the peel. Superficial peels like Glycolic acid Kojic acid peels have very little peeling and hardly any downtime. Aggressive peels like 40% TCA peel or the Obagi Blue peel will have significant peeling and may need a downtime of a week to ten days. The more aggressive peels have much more significant improvement in the skin.

HOW MANY TYPES OF CHEMICAL PEELS ARE THERE?

There are number of chemical peels depending upon their chemical constitution and the amount of peeling that will happen. Superficial peels like Glycolic acid and Kojic acid Salicylic acid and weaker strength of TCA.and mixtures of all remove the very superficial layers or the upper layers of skin known as Epeidermis. Very little peeling and usually have a slight lightening effect and leave the skin looking brighter. Medium Strength peels remove the epidermis and upper part of the dermis which is the next skin layer. Jessner peel TCA 20 to 50% peels are examples of medium strength peels. There is significant downtime with significant peeling but much more improvement in the skin. No anesthesia is required but some discomfort and pain is present. Deep strength peels like phenol or higher strength peels require general anesthesia. All of the epidermis and the dermis and some of the subcutaneous tissue is removed. Downtime is much longer. More chances of complications and much more improvement.

WHAT ARE THE SIDE EFFECTS OF CHEMICAL PEELS?

The side effects depend upon the depth of the peel and type of skin being treated. Deeper the peel more side effects. The darker the skin more chances of hype-rpigmentation. Most side effects are pain and burning when the procedure is being performed. Others can be infection, skin burns and blisters hyper or hypo pigmentation and scarring.

WHAT SKIN PREP DO I NEED TO DO FOR A CHEMICAL PEEL?

Preparing the skin with hydroquinone, glycolic acid and Retin A for four to five weeks before the peel gives a deeper and more even peel. They should all be stopped a week before and Retin A should be stopped at least ten days before the peel. Stay out of the direct sun.

WHAT PRECAUTIONS DO I TAKE AFTER A CHEMICAL PEEL?

  • Stay out of the sun or put sun block SPF 35 or more when you go out.
  • Keep the skin moist with moisturizer while it is healing.
  • No picking, pulling or peeling the skin. You can cut the loose skin with scissors if it bothers you.
  • Wash your skin with gentle soap and water. Pat it dry. Do not rub.
  • In darker skin types and deeper peels hydrocortisone 1% cream and in some cases Steroids by mouth are recommended to prevent hyper-pigmentation.

INSTRUCTIONS FOR THE PATIENT

Patients should avoid the use of the following products for three days prior to the peel:

  • Laser hair removal
  • Waxing
  • Depilatory cream
  • Electrolysis

There are certain products that may increase skin sensitivity and s/or cause a stronger reaction to the peel if used up to the day of the peel:

  • Topical retinoids and tretinoin such as Retin-A, Renova, Differin, Tazorac and Refissa
  • Other topical acids or exfoliants that may be drying or irritating to the skin (such as alpha hydroxyl acids , Beta hydroxyl acids, exfoliating masks, retinols, hydroquinone and benzoyl peroxide acne products).
